Job Summary
The Nursing Care Specialist is responsible for coordinating the plan of care with other healthcare professionals involved in care, ensuring continuity of patient care by planning and exchanging information with the physician, agency personnel, patient, family, case manager, and other community resources.
Key Responsibilities:
- Coordinate care with other healthcare professionals to ensure seamless transitions and continuity of patient care.
- Conduct regular assessments to identify patient needs and develop effective care plans.
- Collaborate with the patient, family, and healthcare team to ensure patient-centered care.
- Maintain accurate and timely documentation of patient progress and care provided.
Requirements
A minimum of one year experience as an RN is preferred. License Requirements: Must have current RN licensure in the state of practice, current drivers license, vehicle insurance, and access to a dependable vehicle or public transportation. Current CPR certification is required.
