About the Role
The Registered Nurse in Home and Community Based Services will provide and direct provisions of nursing and personal care to patients in their homes, as prescribed by the physician or as authorized by the state/program.
You'll coordinate the plan of care with other health care professionals involved in care and help achieve and maintain continuity of patient care.
