This position will travel throughout our territory, providing care for a group of hospice patients where they reside, which could include private homes, and skilled nursing facilitates.

- Ability to work well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and HHA leadership
- Work closely with patients on a 1-1 level but get strong support from a patient care team
- Able to work independently and on your own on a day-to-day basis
- Must be willing to drive with your own, reliable transportation, valid driver’s license, and auto insurance
- HHA in the state(s) of New Jersey required
- CPR certified
