Position Purpose
To develop and facilitate comprehensive care management activities for members with mental and behavioral health needs, ensuring high-quality, cost-effective healthcare outcomes through personalized care plans and education.
- Evaluating member needs via phone or in-home visits to identify available resources and recommend tailored care plans that address behavioral health and social determinant needs.
- Performing telephonic, digital, home, and other site visits to assess member needs and collaborate with relevant resources.
- Developing ongoing care plans for members with high acuity levels and identifying necessary providers, specialists, and community resources for mental health and substance use disorders.
- Coordinating between members, caregivers, community resources, and care provider teams to ensure accessible services.
- Monitoring care plans and member status for changes in treatment side effects, complications, and clinical symptoms, and providing recommendations based on identified needs.
- Facilitating care coordination and collaborating with providers to ensure timely access to needed care or services.
- Collecting, documenting, and maintaining member information and care management activities to ensure compliance with current regulations.
- Providing education to members and their families on procedures, healthcare provider instructions, treatment options, referrals, and benefits, addressing behavioral health and social determinant needs.
- Offering feedback to leadership on opportunities to improve care quality and delivery in a cost-effective manner.
50% Travel required.
We have openings in Northwest Kansas, specifically in Barton, Decatur, Ellis, Grove, Jewell, Lane, Logan, Mitchell, Norton, Osborne, Phillips, Rush, Russell, Smith, Thomas, and Gove counties.
Education/Experience
A Master's degree in Behavioral Health or Social Work is required, along with 2-4 years of related experience. Alternatively, a degree from an accredited nursing school with relevant experience can also be considered.
Licenses/Certifications
- Licensed Behavioral Health Professional or RN, as per state contract requirements (e.g., LCSW, LMSW, LMFT, LMHC, LPC, RN with BH experience).
We offer a competitive salary ranging from $55,100 to $99,000 per year, depending on individual qualifications and experience.
