We are seeking a dedicated and compassionate Registered Nurse for our Home Care department. Duties
- Conduct thorough patient assessments to develop and implement individualized care plans.
- Provide direct patient care, including administering medications, wound care, and monitoring vital signs.
- Collaborate with multidisciplinary teams to ensure comprehensive patient management.
- Educate patients and their families on health management, disease prevention, and treatment plans.
- Document all patient interactions and updates accurately in electronic health records.
- Manage infectious disease care protocols as necessary.
- Participate in utilization management processes to optimize patient care delivery.
Experience
- Previous experience in an acute care setting such as an OR or ER is preferred.
- Familiarity with Level II Trauma Center protocols is advantageous.
- Strong knowledge of medical-surgical nursing practices and medical terminology is essential.
- Experience in primary care settings will be considered a plus.
- Proven ability to perform patient assessments effectively and manage complex health needs.
- Background in infectious disease care is desirable.
