Job Title: Case Manager
Department: Case Management
Supervisor: Treatment Director
FLSA Status: Non-Exempt

Summary
The Case Manager at Sunny Glen Children’s Home (SGCH) is responsible for the safe release of foster care children. The Case Manager is required to maintain accurate documentation on case files and compliance with DFPS Minimum Standards Policy and Procedures, and established state, and federal standards. 

Essential Duties and Responsibilities 
•    Maintain a caseload and accurate records in accordance with agency policies and procedures, licensing standards, and/or funding requirements
•    Conduct initial intake of child in order to assist in the orientation process   
•    Monitor child’s services, ensure all required services are delivered, set initial appointments, and communicate appointments (psychological evaluations, CANS, Circle of Support meetings, counseling sessions, dental/medical/vision checkups, school enrollment, and visitations) with appropriate staff and/or CPS
•    Ensure proper completion of assessments and deadlines as required per internal policy and procedures 
•    Coordinate and complete referrals, service planning, and client documentation of services provided (case notes, admission assessments, incident reports, progress notes, service plans, monthly reports, monthly surveys, etc.)
•    Maintain a therapeutic, and trauma-informed view of each child in his/her caseload and help staff develop this perspective
•    Meet with each child on caseload at least once a week
•    Communicate with CPS caseworkers regarding visitations, Individual Service Plans (ISPs), court dates for youth, any medical, dental and school updates on the youth, and any updates in the youth’s case. 
•    Coordinate placement disruptions and complete discharge/transfer paperwork
•    Demonstrate client progress toward risk reduction, achievement of treatment goals and positive case outcomes 
•    In consultation with his/her supervisor, make decisions affecting the safety of the child 
•    Prepare, compile, and submit accurate and timely case files in case management software and continually assess ongoing changes in behavior, circumstances, or conditions that may affect child safety 
•    Complete, alongside Clinical and Residential programs, appropriate discharge planning prior to the child’s discharge 
•    Participate in weekly case staffing with assigned supervisor, program director, unit managers, direct care workers, house parents, lead shift supervisor, and/or other staff/interns 
•    Act as a liaison with stakeholders, including CPS, CASA, and other entities
•    Work some evenings, after hours, weekends, and holidays as needed to assist with campus emergencies, crises, child placements, or ongoing reunification processes as requested by the supervisor
•    Maintain confidentiality in all areas 
•    Attend training and meetings as scheduled 
•    Participate in weekly staff/team meetings and training 
•    Must abide by Sunny Glen Children’s Home's mission of giving nurturing, trauma-informed care to children from difficult backgrounds

•    Provide Aftercare Services in anticipation of discharge by coordinating school enrollment support, medical and mental health providers and tracking of child outcome data relating to the provisions of T3C service packages.  
•    Provide ongoing engagement with the child and families to support transition and to maintain healthy connections.
•    Serve as a Behavior Support Mentor for children, helping them understand the reasons behind their actions and develop strategies to manage their behavior more effectively.

Qualifications

Education and/or Experience 
A bachelor’s degree in social work or other human services field from an accredited college or university 
Two years of documented full-time work experience in a residential child-care operation, or related field of child and family services.

Language Skills 
Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, and the general public.

Certificates, Licenses, Registrations 
Valid Texas Driver's License with an excellent driving record
Have a record of tuberculosis screening showing the employee is free of contagious TB.
