Responsibilities for Social Services Coordinator
As a Social Services Assistant, you will support the social service function in a long-term care and short-term rehabilitation nursing center. You will work closely with the Social Services Manager to welcome and admit new residents and patients, complete documents related psychosocial assessments, care plans, and discharge plans, and ensure a comfortable and overall positive experience is achieved while completing treatment plans. Qualifications for Social Services Coordinator:
- High School Diploma or equivalent required.
- Post-secondary education or work experience in a social service or health related field a plus (such as social services, social work, patient registration, care coordination, or discharge planning).
- Must be familiar with Medical Terminology.
- Must be proficient in the use of personal computer (Windows, Internet, and Applications/Programs navigation)
