What are the responsibilities and job description for the MDS Coordinator | RN position at Sunnyview Nursing Home?

Responsibilities include :

The coordination and completion MDS assessments and care plans for assigned residents.
The maintenance of resident care plans.
Participation in any facility meetings as warranted.
Monitor and Audit clinical documentation to ensure accuracy.

Qualifications :

Licensed in the state of Rhode Island as an RN with long term care experience.

Knowledge of MDS and careplan process.

Minimum of one year of MDS experience in a long-term care facility.

Should possess the ability to be accurate, concise, detail-oriented and able to work cooperatively as a member of the inter-disiplinary team.

Should possess the ability to organize, prioritize, work in a time-sensitive environment, and communicate within the organization.

Should possess excellent assessment and documentation skills.
