Facility rehab team functions as a cohesive team, with a well experienced director providing superb care to our residents.
Manage caseload in all areas of identifying residents through screenings processes, maintains productivity expectations.
Implements new and innovative processes to assist residents in achieving their goals.
Meets with residents and families prior to discharge to educate for follow through post discharge.
