Sunrise Clinical Associates, PLLC is seeking an Intensive In Home Team Lead.

Position: Qualified Professional

Summary: Intensive In-Home (IIH) service is a team approach to address the identified needs of children and adolescents who, due to serious and chronic symptoms of an emotional, behavioral, or substance use disorder, are unable to remain stable in the community without intensive interventions. This service may only be provided to individuals through age 20. This medically necessary service directly addresses the individual’s mental health or substance use disorder diagnostic and clinical needs. The needs are evidenced by the presence of a diagnosable mental, behavioral, or emotional disturbance (as defined by DSM-5, or any subsequent editions of this reference material), with documentation of symptoms and effects reflected in the Comprehensive Clinical Assessment and Person Centered Plan (PCP). This team provides a variety of clinical rehabilitative interventions available 24 hours per day, 7 days per week, 365 days per year. The service is intended to: (1) reduce presenting psychiatric or substance use disorder symptoms, (2) provide first responder intervention to diffuse current crisis, (3) ensure linkage to community services and resources, and (3) prevent out of home placement for the individual.

Qualifications:

Qualified Professional: a graduate of a college or university with a Master’s degree in a human service field and has one year of full-time, post-graduate degree accumulated MH/DD/SAS experience with the population served, or a substance abuse professional who has one year of full-time, post-graduate degree accumulated supervised experience in alcoholism and drug abuse counseling; or

a graduate of a college or university with a bachelor's degree in a human service field and has two years of full-time, post-bachelor's degree accumulated MH/DD/SAS experience with the population served, or a substance abuse professional who has two years of full-time, post-bachelor's degree accumulated supervised experience in alcoholism and drug abuse counseling; or

a graduate of a college or university with a bachelor's degree in a field other than human services and has four years of full-time, post-bachelor's degree accumulated MH/DD/SAS experience with the population served, or a substance abuse professional who has four years of full-time, post-bachelor's degree accumulated supervised experience in alcoholism and drug abuse counseling.

Job Roles, Duties and Responsibilities

  • Ensure IIH is delivered in accordance with NC DMA Clinical Coverage Policy #8A.
  • Provide psycho-education as indicated in the Person Centered Plan.
  • Consults with identified medical (such as primary care and psychiatric) and non-medical providers (for example, the county department of social services (DSS), school, the Department of Juvenile Justice and Delinquency Prevention (DJJDP), engages community and natural supports, and includes their input in the person-centered planning process.
  • Participates in the person-centered planning process.
  • Assist with implementing a home-based behavioral support plan with the youth and his or her caregivers as indicated in the Person Centered Plan.
  • Assist the team leader in monitoring and evaluating the effectiveness of interventions, as evidenced by symptom reduction and progress toward goals identified in the Person

Centered Plan.

  • Participate in ongoing fidelity monitoring through a combination of internal and external monitoring of EBP fidelity
  • Conduct and submit NC TOPPS interviews in accordance with current NC-TOPPS Implementation Guidelines.
  • Adhere to System of Care values and principles in providing a person-centered, strength-based and recovery-focused environment.
  • Participate in scheduled Utilization Review Meetings.
  • Assist with crisis interventions.
  • Assist the team leader in consulting with identified providers, engaging community and natural supports, and including their input in the person-centered planning process.
  • Coordinates and oversees the initial and ongoing assessment activities.
  • Convenes the Child and Family Team for person-centered planning.
  • Provides intensive case management which includes the following:
  • Assessment;
  • Planning;
  • Linkage and referral to paid and natural supports; and
  • Monitoring and follow-up
  • Completes the initial development and ongoing revision of the PCP and ensures its implementation.
  • Ensures linkage to the most clinically appropriate and effective services including arranging for psychological and psychiatric evaluations.
  • Provide and coordinates behavioral health services and other interventions for the youth or other family members with other licensed professionals and Child and Family Team members.
  • Monitors and documents the status of the recipient’s progress and the effectiveness of the strategies and interventions outlined in the PCP.
  • Generate appropriate documentation of services provided and submit in a timely manner as stipulated in agency’s policy.
  • Attend required meetings (i.e. peer, clinical supervision, staff meetings and weekly IIH Team).
  • Attend required trainings to keep abreast of agency, MCO and Division’s regulations, guidelines and procedures.
  • Participates in client chart audits and other QA/QI projects.
  • Perform other managerial duties, tasks and assignments delegated by supervisor.

Core Competencies:

Technical Knowledge – Understands and can articulate technical concepts and information that relate to the disorder(s) treated and the therapies used to provide treatment. Recognize signs and symptoms of mental health needs and co-occurring disorders related to the population served.

Cultural Awareness – Understands and can articulate the psychological, sociological and political aspects of providing MH/DD/SAS services to diverse populations; ability to communicate and provide competent and appropriate services to diverse populations; utilizes appropriate methods for interacting sensitively, effectively, and professionally with persons from diverse cultural backgrounds.

Analytical Skill – to visualize, articulate and solve complex problems and concepts, and make decisions that make sense based on available information.

Decision Making – Ability to synthesize individual’s information from involved agencies or sources, and to formulate and implement an appropriate clinical course of action in collaboration with these agencies and sources; ability to recognize person’s needs and to refer individual to appropriate professionals and resources; makes appropriate decisions and/or referral in a timely manner.

Interpersonal Skills – the ability to establish rapport, openness and trust; ability to interact effectively with individuals, families, and involved agencies to promote active treatment and prevention.

Communication Skills – ability to effectively formulate and clearly articulate ideas in both verbal and written form. The ability to effectively interpret and convey verbal and non-verbal information through appropriate means for the population served.

Clinical Skills – ability to successfully assess, evaluate and provide individuals with the treatment and therapies appropriate to the person’s identified needs and conditions. Provide assessment, advice, information and treatment on behalf of population served. Different models will dictate where or to whom the service is provided.

Other Position Characteristics

Accuracy of work by the individual in this position will have a tremendous impact on treatment outcomes for clients and parents/families. Efficient allocation of fiscal resources is mandatory in supporting and realizing the primary goals and objectives of children/adolescent services.
This accuracy requires visual/perceptual, verbal and written communication skills.

Consequences of error can range from inconsequential to severe.

Knowledge of agency’s guidelines, policies, and professional standards for treatment, LME and Division’s regulations is required.

Regular contact with agency’s staff (clinical and administrative); some contact with parents/guardians and extensive contacts with staff from other community agencies, school juvenile justice and LME.

There is minimum physical effort to fulfill the requirements of this position.

The tasks of this position generally will be performed in an office setting.

Basic office equipment, including personal computer and materials used in the community based therapeutic practices.

Visual observations of staff’s interactions with clients and peers are essential for training and ongoing evaluation purposes. Visual observation is also essential to the client’s treatment and proper allocation of resources. Intense mental concentration is necessary for the following: managing a client through crisis, managing staff retention and training, overall supervision of staff, fulfilling medical record requirements and other essential paperwork, communicating with officials in the community, evaluating potential emergency placements, etc.

All possible safety measures will be met to protect clients and other staff if the need should arise.

Experience

Qualified Professional (QP) who has the knowledge, skills and abilities required by the population and age to be served. Individual shall have a minimum of 1 year of documented experience with the child MH/SA population.

Specific Skills Required

  • Skills and competence to establish supportive, trusting relationships with persons being served in the program and respect for client rights and personal preferences in treatment are essential.
  • Excellent communication skills, both oral and written.
  • Ability to establish and maintain professional relationships with staff, community agencies and clients.
  • Efficient time management skills.

  • Alternatives to Restrictive Interventions (NCI)
  • Trainings within first 30 days of Employment
  • 3 hours - Intensive In-Home service definition
  • 3 hours – Crisis Response
  • 3 hours – PCP Instructional Elements (Team Leader and QP responsible for PCP development)
  • Trainings within first 90 days of Employment
  • 12 hours of Person Centered Thinking
  • 13 hours of Introductory Motivational Interviewing
  • 11 hours of Introduction to System of Care
  • 24 hours of Cognitive Behavioral Therapy
  • 24 hours (a minimum of 3 days) in one of the designated therapies, practices or models to the population(s) to be served by each IIH Team (these shall be selected by Sunrise Clinical Associates that are referenced in the agency’s procurement contract).

NOTE: Modalities must be in one of the following: Strengthening Families Program, Eco-Systemic Structural Family Program and Trauma-Focused Cognitive Behavioral Therapy.

  • 10 hours of follow-up training and ongoing continuing education for fidelity to chosen modality (this shall be completed annually).
  • Internal and external trainings on selected Evidence Based Practices (as stipulated in procurement contract)
  • Other designated trainings required by the agency to enhance job knowledge and performance
  • Other designated trainings required by the regulatory entities
