Sunrise Clinical Associates, PLLC is seeking an Intensive In Home Team Lead in Fayetteville, NC.
Position:Licensed Professional (Team Lead)
Summary:The Intensive In-Home (IIH) service is a team approach designed to address the identified needs of children and adolescents who, due to serious and chronic symptoms of an emotional, behavioral, or substance use disorder, are unable to remain stable in the community without intensive interventions. This service may be provided to beneficiaries through age 20. This medically necessary service directly addresses the beneficiary’s mental health or substance use disorder diagnostic and clinical needs. The needs are evidenced by the presence of a diagnosable mental, behavioral, or emotional disturbance (as defined by DSM-5, or any subsequent editions of this reference material), with documentation of symptoms and effects reflected in the Comprehensive Clinical Assessment or Diagnostic Assessment and the Person-Centered Plan (PCP). This team provides a variety of clinical rehabilitative interventions available 24 hours per day, 7 days per week, 365 days per year. The service is intended to accomplish the following: (1) reduce presenting psychiatric or substance use disorder symptoms, (2) provide first responder intervention to diffuse current crisis, (3) ensure linkage to community services and resources, and (4) prevent out of home placement for the beneficiary. The team approach involves structured, face-to-face, scheduled therapeutic interventions to provide support and guidance across multiple functional domains including emotional, medical and health.
Qualifications:
Licensed Professional who is an individual who holds a license, provisional license, certificate, registration or permit issued by the governing board regulating a human service profession. Licensed Professional who has the knowledge, skills, and abilities required by the population and age to be served. Associate Level professional actively seeking licensure may serve as the team leader conditional upon being fully licensed within 30 months from the effective date of the DMA Clinical Coverage Policy #8A. For Associate Level licensed team leaders hired after the effective date of the DMA Clinical Coverage Policy #8A, the 30-day month timeline begins at date of hire.
Job Roles, Duties and Responsibilities
Provides individual and family therapy for each beneficiary served by the team.
- Designates the appropriate team staff such that specialized clinical expertise is applied as clinically indicated for each beneficiary.
- Provides and coordinates the assessment and reassessment of the beneficiary’s clinical needs.
- Provides clinical expertise and guidance to the IIH team members in the team’s interventions with the beneficiary.
- Provides the clinical supervision of all members of the team for the provision of this service. An individual supervision plan is required for all IIH team members.
- Ensures intensive case management is rendered by the team to include the following:
- Assessment
- Planning
- Linkage and referral to paid and natural supports
- Monitoring and follow up
- Assists with crisis interventions.
- Provides psycho-education as indicated in the PCP (psycho-education imparts information about the beneficiary’s diagnosis, condition, and treatment to the beneficiary, family, caregivers, or other individuals involved with the beneficiary’s care).
- Responsible for monitoring and documenting the status of the recipient’s progress and the effectiveness of the strategies and interventions outlined in the PCP.
- Consults with identified medical (such as primary care and psychiatric) and non-medical providers (for example, the county department of social services (DSS), school, the Department of Juvenile Justice and Delinquency Prevention (DJJDP), engages community and natural supports, and includes their input in the person-centered planning process.
- Effectively engages, including cultural sensitivity, when providing services in the family’s living environment.
- Provides clinical expertise and guidance to the IIH team members in the team’s interventions with the recipient.
- Coordinates and oversees the initial and ongoing assessment activities.
- Convenes the monthly Child and Family Team for person-centered planning.
- Completes the initial development and ongoing revision of the PCP and ensures its implementation.
- Oversees the implementation in selective family-focused Evidence Based Practices (EBPs) within your Intensive In-Home Services (IIH) team for (Alliance catchment area). EBPs include the following: (1) Strengthening Families Program (SFP), (2) Eco-systemic Structural Family Program (ESFT) and (3) Trauma Focused Cognitive Behavioral Therapy (TF-CBT).
- Ensures models are implemented in accordance with expectations and specific requirements for the staff you supervise, attends internal trainings, and documents ongoing fidelity monitoring as stipulated in the selective family-focused Evidence Based Practices (EBPs).
- Ensures linkage to the most clinically appropriate and effective services including arranging for psychological and psychiatric evaluations.
- Provides and coordinates behavioral health services and other interventions for the child/youth or other family members with other licensed professionals and Child and Family Team members.
- Monitors and documents the status of the recipient’s progress and the effectiveness of the strategies and interventions outlined in the PCP.
- Periodically audit service notes to monitor quality and substantiation of amount billed. Utilizes note audit forms/tools and provide feedback/technical assistance of findings to QA/QI Director and other managerial staff. Issue a deadline to correct/revise service notes, when applicable.
- Participates in the person-centered planning process.
- Assists with implementing a home-based behavioral support plan with the child/youth and the child/youth caregivers as indicated in the PCP.
- Provides psycho-education as indicated in the PCP.
- Monitors and evaluate the effectiveness of interventions, as evidenced by symptom reduction and progress toward goals identified in the PCP.
- Assists with crisis interventions.
- Consults with identified providers, engaging community and natural supports, and including their input in the person-centered planning process.
- Generates appropriate documentation of services provided and submit in a timely manner as stipulated in agency’s policy and Records Management and Documentation Manual.
- Completes 90 day and annual performance evaluation for assigned team members.
- Attends required meetings (i.e. peer review, clinical supervision, staff meetings, and Team Lead meetings).
- Participates in Utilization Reviews and other Clinical Meetings when necessary.
- Monitors continuously to ensure titration occur (when beneficiary needs more or fewer services, the team provide services based on that level of need).
- Ensures NC TOPPS is completed during required intervals, as referenced in the NC TOPPS Implementation Guidelines.
- Facilitates weekly Intensive In-Home team meetings.
- Prepares adequate documentation and attends required Utilization Reviews.
- Appropriately prepares, documents and monitors transition/discharge process; upon actual discharge ensure paperwork is completed in its entirety.
- Submits documentation for weekly Intensive In-Home team meetings, monthly Child & Family Team Meetings, monthly supervision logs and client specific clinical outcomes.
- Attends required trainings to keep abreast of agency, MCOs and Division’s regulations, guidelines and procedures.
- Participates in client chart audits and other QA/QI projects
- Performs other managerial duties, tasks and assignments delegated by supervisor or Executive Management staff.
NOTE: This position must be credentialed by Alliance Behavioral Healthcare and Cardinal Innovations Healthcare MCOs.

Accuracy of work by the individual in this position will have a tremendous impact on treatment outcomes for clients and parents/families. Efficient allocation of fiscal resources is mandatory in supporting and realizing the primary goals and objectives of children/adolescent services. This accuracy requires visual/perceptual, verbal and written communication skills.
Consequences of error can range from inconsequential to severe.
Knowledge of agency’s guidelines, policies, and professional standards for treatment, MCOs and Division’s regulations is required.
Regular contact with agency’s staff (clinical and administrative); some contact with parents/guardians and extensive contacts with staff from other community agencies, school juvenile justice and MCO.
There is minimum physical effort to fulfill the requirements of this position.
The tasks of this position generally will be performed in an office setting.
Basic office equipment, including personal computer and materials used in the community based therapeutic practices.
Knowledge of how to utilize agency’s web-based Checkpoint Integrity Support for note generation and addendums.
Visual observations of staff’s interactions with clients and peers are essential for training and ongoing evaluation purposes. Visual observation is also essential to the client’s treatment and proper allocation of resources. Intense mental concentration is necessary for the following: managing a client through crisis, managing staff retention and training, overall supervision of staff, fulfilling medical record requirements and other essential paperwork, communicating with officials in the community, evaluating potential emergency placements, etc.
All possible safety measures will be met to protect clients and other staff if the need should arise.
Experience
Team Leader must be a Licensed Professional or Associate Level Licensed Professional who has the knowledge, skills and abilities required by the population and age to be served. Individual shall have a minimum of 1 year of documented experience with the MH/SA population served.
