Medical Records Clerk:
The Medical Records Clerk is responsible for maintaining the files included in a patient’s health information chart, including medical history, symptoms, examination results, diagnostic tests, treatment methods, and other services.
Position Summary:
With a Quality, Customer First, and Compassionate approach, The Medical Records Clerk will:
- Organize and manage patient’s health information data.
- Maintain patient health information which includes symptoms, medical history, results of examinations, reports of x-rays and lab tests, observations, medical or surgical interventions, diagnoses, and treatment plans.
- Ensure paperwork is properly filled out. Make sure patient medical charts are complete and all forms are completed and properly identified and signed.
- Organize and evaluate records for completeness, accuracy, and compliance with regulations.
- Protect the security of medical records to ensure that confidentiality is maintained. Maintain HIPAA standard for patient confidentiality.
- Regularly communicate with physicians and other health care providers to clarify diagnoses or to obtain additional information.
- Maintain electronic health records (EHR) databases. Make sure all necessary information is in the computer. Scan appropriate documents in EHR.
- Respond to requests for information from files according to established policies and procedures.
- Complete medical record release requests in a timely fashion.
Minimum Qualifications:
- High School Diploma or general education degree (GED),
- Certificate or Associate Degree in Health Information Technology or Medical Record Technology
- 1-2 years’ experience and/or training; or equivalent combination of education and experience.
- Bilingual English/Spanish Preferred
