Position Summary: The Housing Case Manager at Sunrise is responsible for providing comprehensive case management services to individuals and families experiencing homelessness, including assisting clients in securing permanent housing and managing their housing vouchers. The role requires strong skills in navigating information systems, working with diverse populations, and maintaining detailed documentation in the HMIS database. Bilingual candidates and those with a background in social work, particularly with supervisory experience, are strongly preferred.
Position Compensation: $23 - $25 per hour, 25-30 hours per week Monday - Friday, paid vacation, sick and holiday time
Position Qualifications:
- Bachelor’s degree in social services, business or public health or related field preferred
- Current or previous HMIS licensure preferred
- Bilingual (Spanish) is strongly preferred
- Knowledge of homelessness prevention, crisis intervention, and referral systems.
- Excellent communication and interpersonal skills, with the ability to engage effectively with diverse stakeholders.
- Familiarity with database management systems and information dissemination technologies is preferred.

Position Responsibilities:
- Provide comprehensive, timely, and reliable case management services to address clients’ needs
- Access housing barriers of individuals and families experiencing homelessness to determine housing and service needs
- Assist clients with securing affordable, permanent housing including handling the housing search, working with landlords, and advocating for clients
- Manage case data by documenting client meetings, goals and outcomes in the Service Point database system
- Develop and maintain relationships with local agencies and landlords
- Assist clients in managing their Section 8 housing voucher
- Provide regular face-to-face meetings to assure clients’ progress toward self-sufficiency
- Link clients with community agencies for additional support (i.e. food pantries, SS applications, food stamps, etc.)
- Work creatively and professionally with other case managers and Housing Lead to determine best integration of activities and resources for clients
- Provide strength-based crisis intervention and problem solving for clients
- Utilize trauma-informed care when working with clients with an adaptable & compassionate attitude
- Attend and participate in ECHO By-Name Staffing meetings, Sunrise all-staff meetings & Housing Team meetings
- Help develop relationships with landlords
Position Considerations:
- Support EHV/PSH staff as requested with service provision.
- Work with a caseload of 5-10 clients.
- Meet caseload minimums and report in HMIS.
- Actively listen to clients stated needs and make appropriate resource referrals.
- Consistently and accurately enter information into the HMIS database.
- Assist with collection and maintenance of data for statistical and programming purposes.
- Navigate and utilize digital information on a daily basis.
- Collaborate regularly with different Sunrise teams and outside service providers.
- Occasionally attend in-person meetings.
- Maintain a safe work environment and confidentiality at all times.
- Be proactive, creative, and flexible in determining, evaluating, researching, and resolving issues.
- Organize and prioritize multiple activities to meet all external and internal deadlines

Work Environment:
- Supervision of interns (stipend of up to $1,000 is available) for BSW and MSW applicants.
- This is a hybrid position.
- Sunrise’s operation is rapidly expanding and building. It is very possible that this position could shift or grow if the applicant desires.
- Working as part of a team and flexibility with roles is important in Sunrise’s structure.
- This is a new and developing program. Opportunities to take on special projects and contribute to quality improvement efforts are available but not required.
- Due to licensing restrictions, job duties may vary for the first few months while licensure is acquired.
