About the Job
We are seeking an experienced LPN to join our team as a Care Manager at Sunrise Senior Living. As a Care Manager, you will play a crucial role in ensuring the highest quality of care and services for our residents. Your primary responsibilities will include administering medication and treatments in a safe and organized manner.
Key Responsibilities
- Administer medications and treatments to residents in accordance with their individualized care plans.
- Document all medication administration in the resident's medical record.
- Conduct Service and Health Updates as directed by the Resident Care Director (RCD).
- Participate in the development and implementation of Individualized Service Plans (ISPs) and monthly updates.
- Transcribe orders and maintain accurate records of resident information.
Qualifications and Requirements
- A valid Ohio LPN license.
- CPR and First Aid certification.
- Ability to make choices and decisions that promote the best interest of residents.
- Effective communication and interpersonal skills.
- Good judgment and problem-solving skills.
- Able to work independently and as part of a team.
