Job Description
As an LPN at Sunrise, you will play a vital role in delivering exceptional care to our residents. You will be responsible for conducting regular wellness visits, monitoring medication and treatment profiles, and collaborating with the Resident Care Director (RCD) to ensure seamless care delivery.
Responsibilities
- Conduct monthly wellness visits for all residents
- Monitor and follow up with residents who have experienced any fall, accident, or incident
- Document all pertinent information in the resident wellness file
- Maintain communications with resident's family and/or responsible party to ensure medical needs for the resident are being met
- Contacts resident's attending physician when necessary and/or upon family request
Qualifications
- Graduate of approved college/school of nursing
- Current state license as a professional Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN)
- Minimum one year experience in home health, assisted living, hospital, or long-term care environment
- Demonstrated knowledge of nursing practices, techniques, and methods applied to health and wellness resident care
