Responsibilities:
- Lead and manage the health and wellness of our residents, including medication management and clinical quality tracking.
- Collaborate with cross-functional teams to ensure seamless care coordination and service delivery.
- Participate in Quality Assurance and Performance Improvement committees, driving initiatives to improve resident care outcomes.
Requirements:
- Graduate of an accredited college or school of nursing with a current state license as a professional LVN/LPN or RN.
- Minimum two (2) years nursing experience, preferably in home health, assisted living, or long-term care.
- Certified in CPR and First Aid.
