Your Key Responsibilities
">You will be responsible for participating in the development of Individualized Service Plans (ISP)/Individualized Care Plans (ICP) and monthly updates. You will also be required to attend and actively participate in daily Cross Over meetings facilitated by the LCM.
Requirements
">To be successful in this role, you must have a high school diploma or equivalent, and you must be certified in CPR and First Aid. You should also have previous experience working with seniors, and a desire to serve and care for them.
