\\Responsibilities

- Conduct Regular Assessments: Identify on-going needs and services of residents, ensuring timely delivery of high-quality resident services. \\
- Collaborate with Resident Care Director: Assist in conducting resident assessments at time of move-in, 30 days post-move-in, semi-annually, and with any change in status. \\
- Maintain Accurate Records: Document all pertinent information in the resident wellness file, ensuring compliance with federal, state, and local regulations. \\
- Communicate Effectively: Maintain communications with resident's family and/or responsible party to ensure medical needs are being met. \\
- Provide Clinical Support: Assist community team members as needed, ensuring seamless delivery of care services. \\
Qualifications
\\- \\
- Graduate of Approved College/School of Nursing: Possess a current state license as a professional Registered Nurse (RN). \\
- Minimum of One Year Experience: Showcase expertise in home health, assisted living, hospital, or long-term care environments. \\
- Strong Assessment Skills: Demonstrate knowledge of nursing practices, techniques, and methods applied to health and wellness resident care. \\
- Excellent Communication Skills: Possess written and verbal skills for effective communication and the ability to teach and guide team members. \\
- Competent in Organizational Time Management: Show proficiency in computer skills, Microsoft Office, and Sunrise applications. \\
