Job Description:
The Lead Care Manager serves as a key leader in our community, working closely with the department care coordinator to develop and implement effective care plans for our residents. This role requires strong leadership skills, attention to detail, and a passion for delivering exceptional care and services.
Key Responsibilities:
- Lead by Example: Demonstrate a positive, professional, and friendly image through action, words, and dress, and provide feedback to the department care coordinator for care manager annual performance reviews.
- Care Coordination: Communicate any observed or suspected resident change of condition to the department care coordinator, and assist care coordinators when required in development and/or updates to ISPs and communicating with residents and families.
- Problem-Solving and Conflict Resolution: Prevent and handle conflict by working with the team towards a solution, and communicate issues to the department care coordinator when necessary.
