Job Title: LPN Medication Care Manager

As an LPN Medication Care Manager, you will be responsible for providing high-quality care and services by administering medication and treatments in a safe and organized manner.
Responsibilities:
- Administer medications and treatments to residents while maintaining accurate documentation and adherence to federal, state, and local laws and regulations.
- Conduct Service and Health Updates as directed by the Resident Care Director (RCD).
- Participate in the development of Individualized Service Plans (ISP)/Individualized Care Plans (ICP) and monthly updates.
- Maintain confidentiality of all resident information, including medication administration records.
- Notify RCD of any resident and/or family concerns through the LEAD process.
Requirements:
- Valid Ohio nursing license.
- CPR and First Aid certification.
- Previous experience working with seniors is preferred.
- Ability to make choices and decisions in the best interest of residents.
- Ability to remain calm and composed in difficult situations.
