About the Role
The LPN Medication Care Manager plays a vital part in our community's healthcare team. Your primary responsibilities will include administering medications, documenting medication administration, conducting SHUs, and providing resident care while adhering to federal, state/provincial, and local laws, standards, and regulations.
In addition to these duties, you will also be expected to participate in daily Cross Over meetings, conduct Service and Health Updates as directed by the RCD, and develop Individualized Service Plans (ISP)/Individualized Care Plans (ICP) and monthly updates.
