Job Description
The LPN Medication Care Manager is responsible for providing high-quality care and services to our residents. This includes administering medications, documenting medication administration, and collaborating with other healthcare professionals.
Key Responsibilities:
- Medication Administration: Administer medications and treatments in a safe and organized manner.
- Documentation: Maintain accurate and up-to-date records of medication administration.
- Communication: Collaborate with Resident Care Directors and Wellness Nurses to ensure timely medication updates.
- Service and Health Updates: Conduct Service and Health Updates as directed by RCD.
- Individualized Service Plans: Participate in the development of ISP and monthly updates.
Requirements:
- Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) credential.
- CPR and First Aid certification.
- Ability to make choices and decisions in the best interest of residents.
- Strong communication and problem-solving skills.
