Responsibilities
- Resident Care: Provide high-quality nursing care to our residents, including conducting monthly wellness visits, monitoring vital signs, and administering medications as prescribed.
- Team Leadership: Collaborate with our Resident Care Director to conduct resident evaluations, gather data, and develop individualized care plans.
- Communication: Maintain open communication with residents' families and healthcare providers to ensure seamless care transitions and address any concerns or issues that may arise.
Requirements
- Nursing Degree: Must possess a current state license as a professional Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN) and a graduate degree from an approved college/school of nursing.
- Caregiving Experience: Minimum one year experience in home health, assisted living, hospital, or long-term care environment, with knowledge of good assessment skills and ability to handle multiple priorities.
- Computer Skills: Proficient in computer skills, Microsoft Office, and Sunrise applications with the ability to learn new applications.
