About the Role:

Responsibilities:
- Conduct comprehensive assessments of residents' health status and identify areas for improvement.
- Develop and implement effective care plans that address residents' physical, emotional, and social needs.
- Collaborate with other healthcare professionals to ensure seamless care transitions and continuity of service.
- Provide education and support to residents and their families on healthy lifestyle choices and disease management.
- Maintain accurate and up-to-date records of resident care, including progress notes and treatment plans.
- Participate in quality improvement initiatives to enhance the overall quality of care provided at Sunrise Senior Living.
Requirements:
- A graduate degree in nursing or a related field.
- A valid state license as a Nurse Practitioner (NP) or Family Nurse Practitioner (FNP).
- A minimum of one year of experience in gerontological nursing or a related field.
- Demonstrated expertise in assessing and addressing the complex needs of older adults.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
