Job Description
As a Care Manager at Sunrise, you will be responsible for building meaningful relationships with our residents and their families. You will provide assistance with activities of daily living, attend to individual care needs, and get to know the unique preferences and personalities of our residents.
You will also be responsible for notifying management of changes in condition and recommending adjustments in the level of care and service. Additionally, you will participate in the development of Individualized Service Plans (ISPs) and monthly updates.
