Job Summary

Key Responsibilities
- Develop and implement individualized service plans for residents.
- Collaborate with the care team to ensure accurate documentation and confidentiality of resident information.
- Lead daily cross-over meetings to discuss resident needs and concerns.
- Ensure compliance with regulatory requirements and company policies.
Requirements
- LPN, LVN, or state-specific licensed nurse credential.
- CPR and First Aid certification.
- Prior experience working with seniors is preferred.
- Ability to make choices and decisions in the best interest of residents.
