We are seeking a highly skilled and compassionate Wellness Nurse to join our team at Sunrise Senior Living. As a Wellness Nurse, you will be responsible for identifying ongoing needs and services of residents, conducting monthly wellness visits, and providing clinical support to our team members.
Key Responsibilities
- Identify ongoing needs and services of residents to promote high-quality resident services
- Conduct monthly wellness visits for all residents
- Monitor and follow up with residents who have experienced falls, accidents, or incidents; document and report as required and directed
Requirements
- Graduate of an approved college/school of nursing
- Current state license as a professional Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN)
- Minimum one year experience in home health, assisted living, hospital, or long-term care environment
