Job Description
The RN Wellness Nurse plays a vital role in ensuring the well-being of our residents, working closely with the Resident Care Director to identify ongoing needs and services. This position requires a registered nurse with a passion for delivering high-quality care and promoting healthy aging.
Responsibilities
- Identify on-going needs and services of residents, promoting the highest quality of resident services.
- Ensure proper follow-through, assessment, and documentation for residents with a change in healthcare services and status.
- Assist the Resident Care Director in conducting resident assessments at time of move-in, 30 days post-move-in, semi-annually, and with any change in status.
- Conduct monthly wellness visits for all residents, assessing and monitoring their health status.
- Maintain effective communication with residents' families and healthcare providers, ensuring seamless transitions and coordination of care.
Requirements
- Graduate of an approved college/school of nursing, holding a current state license as a professional Registered Nurse (RN).
- Minimum one year experience in home health, assisted living, hospital, or long-term care environment.
