Job Description
We are seeking a highly skilled and compassionate LPN Medication Care Manager to join our team at Sunrise Senior Living. As a key member of our care team, you will be responsible for administering medications and treatments in a safe and organized manner.
Responsibilities:
- Administer medications and treatments according to physician orders and facility policies
- Document medication administration and any resident concerns or issues
- Participate in daily Cross Over meetings to discuss resident care and coordinate with the care team
- Conduct Service and Health Updates as directed by the Resident Care Director
- Transcribe orders and maintain accurate records
Requirements:
- LPN or LVN license in good standing
- Previous experience working with seniors preferred
- Ability to make choices and decisions in the best interest of residents
- Good communication and interpersonal skills
- Ability to work independently and as part of a team
