Responsibilities
- We are seeking a Licensed Practical Nurse (LPN) Medication Care Manager to administer medication and treatments in a safe and organized manner.
The successful candidate will be responsible for providing high-quality care and services by administering medication, documenting administration, conducting Service and Health Updates, and providing resident care while upholding Sunrise's Mission and values.
Key Responsibilities:
- Administer medications, document administration, conduct Service and Health Updates, and provide resident care.
- Partner with the Resident Care Director (RCD) and Wellness Nurse to receive medication updates and ensure timely documentation.
- Document medication passes, initials, and refusal/missed doses according to procedures.
- Maintain confidentiality of resident information, including medication records.
Resident Care Services
- Notify the RCD of any resident or family concerns through the LEAD process.
- Attend Cross Over meetings facilitated by the Lead Care Manager (LCM).
- Conduct Service and Health Updates as directed by the RCD.
- Participate in developing Individualized Service Plans (ISP)/Individualized Care Plans (ICP) and monthly updates.
Risk Management and General Safety
- Ensure compliance with OSHA/British Columbia Workers Compensation Act and Occupational Health and Safety Regulation requirements.
- Practice safety procedures, including Personal Protective Equipment (PPE), fire extinguishers, Safety Data Sheets (SDS), and Lockout Tagout procedures.
Training and Contributing to Team Success
- Actively participate as a team member and commit to working toward team goals.
- Demonstrate the Team Member Credo in daily interactions.
Qualifications
- Licensed Practical Nurse (LPN) or state/province-specific licensed nurse credential.
- Maintain certifications, such as CPR and First Aid, as required by Sunrise and state/provincial regulations.
The ideal candidate will possess strong communication and problem-solving skills, ability to make choices and decisions, and react calmly in difficult situations. Previous experience working with seniors is preferred but not required.
