Job Summary

The Senior Care Nurse will play a vital role in promoting the overall well-being of our residents. This includes conducting regular wellness visits, monitoring resident health status, and ensuring timely documentation of all relevant information.

Responsibilities and Qualifications:

Conduct monthly wellness visits for all residents.Graduate of an approved college/school of nursing.
Monitor and follow up with residents who have experienced any falls or incidents.Current state license as a professional Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN).
Document all pertinent information in the resident's wellness file.Minimum one year experience in home health, assisted living, hospital, or long-term care environment.
