As a Senior Care Specialist, you will be responsible for conducting monthly wellness visits, assessing and documenting resident health status, and collaborating with other team members to promote infection control standards.

Responsibilities
- Conduct monthly wellness visits for all residents
- Assess and document resident health status
- Collaborate with other team members to promote infection control standards
- Provide clinical support and assistance to community team members as needed
Qualifications
- Graduate of an approved college or school of nursing
- Maintains a current state license as a professional Registered Nurse (RN)
- Minimum of one year experience in home health, assisted living, hospital, or long-term care environment
