Responsibilities:
- Care Coordination: Work closely with the Resident Care Director (RCD) to conduct thorough assessments of new residents, monitor their health status, and provide personalized care plans.
- Health Promotion: Conduct monthly wellness visits for all residents, assess their needs, and develop strategies to promote healthy lifestyles.
- Fall Prevention: Assess, monitor, and follow up with residents who have experienced falls or accidents, documenting and reporting as required.
- Maintenance of Records: Maintain accurate and up-to-date records of resident medical information, including vital signs and medication profiles.
Requirements:
- Education: Graduate of an approved college/school of nursing.
- Licensure: Current state license as a professional Registered Nurse (RN).
- Experience: Minimum one year experience in home health, assisted living, hospital, or long-term care environment.
- Skills: Excellent communication and interpersonal skills, ability to work independently and as part of a team.
