As a Wellness Nurse at Sunrise, you will play a crucial role in ensuring the overall health and well-being of our residents. This includes conducting regular wellness visits, monitoring vital signs, and providing medical support as needed.
Responsibilities
- Maintain accurate records of resident care, including medical history, medication administration, and any changes in condition.
- Provide education to residents, family members, and staff on various healthcare topics, such as fall prevention, nutrition, and disease management.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans for each resident.
- Conduct regular assessments of residents' physical and emotional needs, identifying areas for improvement and making recommendations for interventions.
- Participate in quality improvement initiatives, contributing to the development and implementation of policies and procedures to enhance resident care.
Qualifications
- A valid LPN or LVN license in good standing with the state licensing authority.
- At least one year of experience working in a long-term care setting, preferably in a nursing or caregiving role.
- Demonstrated expertise in gerontology, rehabilitation, and palliative care.
- Excellent communication and interpersonal skills, with the ability to effectively interact with residents, families, and healthcare professionals.
- Ability to work independently, prioritize tasks, and manage multiple responsibilities simultaneously.
