Sunrise Senior Living Job Summary: We are seeking a highly skilled LPN to join our team as a Medication Care Manager. In this role, you will be responsible for providing high-quality medication management services to our residents, including administering medications and treatments in a safe and organized manner.

You will work closely with the care team to ensure seamless delivery of care, and maintain accurate records of medication administration and resident interactions. Your attention to detail and commitment to confidentiality will be essential in ensuring the highest standards of care are maintained.

Responsibilities:

• Administer medications and treatments to residents.
• Conduct Service and Health Updates as directed by the Resident Care Director.
• Participate in daily Cross Over meetings and contribute to the development of Individualized Service Plans (ISPs).
• Maintain accurate records of medication administration and resident interactions.

Requirements:

• Licensed Practical Nurse (LPN) or state-specific equivalent.
• Certifications in CPR and First Aid.
• Previous experience working with seniors preferred.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.
