Responsibilities:
- Identify ongoing needs and services of residents, promoting the highest quality resident services delivery.
- Ensure proper follow-through, participate in evaluation and documentation for residents with a change in healthcare services and status.
- Assist the Resident Care Director in conducting resident evaluations and gathering data at move-in, 30 days post-move-in, semi-annually, and with any change in status.
Qualifications:
- Graduate of an approved college or school of nursing.
- Maintain a current state license as a professional Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN).
- Minimum one year experience in home health, assisted living, hospital, or long-term care environment.
