As an LPN Medication Care Manager, you will be an integral part of our care team, working closely with residents, family members, and other healthcare professionals to ensure the best possible outcomes.

Responsibilities and Requirements:

• Administer medications and treatments to residents as prescribed by their physicians
• Document all medication administrations in resident records
• Conduct Service and Health Updates as directed by the RCD
• Participate in the development of Individualized Service Plans (ISPs) and monthly updates
• Transcribe orders from physicians and other healthcare professionals
