Job Description
The LPN Care Coordinator is responsible for providing high-quality care and services to our residents. This includes administering medication and treatments in a safe and organized manner. The successful candidate will be able to work independently and as part of a team to ensure the best possible outcomes for our residents.
- Administers, assists with, and observes medications and treatments for each resident using the medication administration record and the Six Rights of Medication Pass
- Documents and initials as medications are given and ensures that appropriate documentation is completed for refusal or missed doses
- Maintains confidentiality of all resident information including resident medication among other residents
