Job Overview
We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team as a Wellness LPN Professional. In this role, you will be responsible for providing exceptional care to our residents, ensuring their physical and emotional needs are met.
Responsibilities:
- Conduct Regular Wellness Visits: You will conduct monthly wellness visits for all residents, monitoring their health status and documenting any changes or concerns.
- Develop and Implement Care Plans: You will work with the Resident Care Director to develop and implement individualized care plans that cater to each resident's unique needs and preferences.
- Maintain Accurate Records: You will maintain accurate and up-to-date records of resident assessments, care plans, and progress notes.
- Collaborate with Healthcare Team Members: You will work collaboratively with other healthcare team members, including physicians, therapists, and social workers, to ensure comprehensive and coordinated care.
Requirements:
- Graduate of an approved college/school of nursing
- Current state license as a professional Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN)
- Minimum of one year experience in home health, assisted living, hospital, or long-term care environment
