Responsibilities:
- Provide expert caregiving services to residents, including identifying ongoing needs and collaborating with the interdisciplinary team.
- Document and communicate changes in resident healthcare services and status accurately and promptly.
- Participate in conducting comprehensive resident assessments at various intervals.
- Develop and implement personalized care plans tailored to each resident's unique needs.
- Build strong relationships with residents, families, and staff through effective communication and empathy.
Requirements:
- Current RN licensure and degree from an accredited nursing program.
- Proven experience in caregiving, preferably in home health, assisted living, or long-term care.
- Exceptional communication, interpersonal, and problem-solving skills.
- A passion for delivering high-quality care and services to seniors.
