Job Title: Wellness Nurse
We are seeking a compassionate and skilled Wellness Nurse to join our team at Sunrise Senior Living. As a Wellness Nurse, you will play a vital role in ensuring the overall well-being of our residents.
Key Responsibilities:
- Identify ongoing needs and services of residents to promote high-quality resident services.
- Ensure proper follow-through, assessment, and documentation for residents with changes in healthcare services and status.
- Conduct monthly wellness visits for all residents and assess, monitor, and follow up on residents who have experienced falls or accidents.
- Maintain communication with residents' families and responsible parties to ensure medical needs are being met.
Requirements:
- Graduate of an approved college/school of nursing.
- Current state license as a professional Registered Nurse (RN).
- Minimum of one year experience in home health, assisted living, hospital, or long-term care environment.
