Job Description
Sunrise Senior Living is seeking a skilled and compassionate Wellness Nurse to join our team. As a key member of our healthcare staff, you will be responsible for ensuring the overall well-being of our residents, providing exceptional care and support to empower them to live longer, healthier, and happier lives.
Key Responsibilities
• Identify on-going needs and services of residents to promote the highest quality resident services
• Participate in evaluation and documentation for residents with a change in health care services and status
• Assist Resident Care Director (RCD) in conducting resident evaluations and data gathering information at time of move in, 30 days post move in, semi-annually and with any change in status
• Conduct monthly wellness visits for all residents
Qualifications
• Graduate of approved college/school of nursing
• Minimum of one (1) year experience in home health, assisted living, hospital, or long-term care environment
• Demonstrates knowledge of nursing practices, techniques and methods applied to health and wellness resident care, and federal, state, and local regulations
• Possesses written and verbal skills for effective communication and the ability to teach and guide team members to provide quality clinical care
