The Wellness Nurse is responsible for ensuring the overall health and well-being of our residents, working closely with the Resident Care Director to promote quality care and services. Key responsibilities include identifying ongoing needs, participating in evaluations, documenting resident information, and maintaining effective communication with healthcare providers and family members. The successful candidate will possess strong assessment skills, excellent written and verbal communication skills, and the ability to work effectively in a fast-paced environment.
Requirements and Qualifications
To be successful in this role, the ideal candidate will be a graduate of an approved college or school of nursing, hold a current state license as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN), and have a minimum of one year's experience in a long-term care setting. They will also possess knowledge of good assessment skills, organizational time management skills, and proficiency in computer skills, including Microsoft Office and Sunrise applications.
