Responsibilities:
- Collaborate with the community team to identify ongoing needs and services of residents, promoting high-quality resident services and outcomes.
- Conduct monthly wellness visits for all residents, monitoring and following up on any incidents or falls, and maintaining accurate records of resident wellness files.
- Communicate effectively with families regarding medical needs and collaborate with the Resident Care Director (RCD) to promote infection control standards and best practices.
