We are seeking an experienced Nurse to join our team at Sunrise Senior Living. As a Wellness Nurse, you will have the opportunity to make a real difference in the lives of our residents, working closely with them and their families to promote their health, wellbeing, and quality of life.
About the Role:
The Wellness Nurse is responsible for conducting resident assessments, monitoring their health and wellbeing, and collaborating with the care team to develop personalized care plans. You will also be responsible for identifying on-going needs and services of residents, ensuring proper follow through, assessment and documentation for residents with a change in healthcare services and status.
Key Responsibilities:
- Assess and monitor resident health and wellbeing
- Develop and implement individualized care plans
- Collaborate with the care team to ensure high-quality care and support
- Maintain accurate records of resident care and progress
Requirements:
- Degree in Nursing (BSN or ADN)
- Current state license as a Registered Nurse (RN)
- Minimum one year experience in home health, assisted living, hospital, or long-term care environment
