Job Summary
We are seeking an experienced Licensed Practical Nurse (LPN) to join our team. As a key member of our healthcare team, you will be responsible for developing and implementing individualized nursing care plans, supervising nursing personnel, and participating in direct resident assessment and care.

Responsibilities
• Develop and implement individualized nursing care plans to meet the unique needs of each resident
• Assist in the orientation and supervision of nursing personnel, ensuring they are equipped to provide high-quality care
• Attend to daily operations of the unit, assuming leadership roles as needed to ensure smooth functioning
• Participate in direct resident assessment and care, working collaboratively with other healthcare professionals
• Supervise and coordinate nursing personnel in providing direct resident care, adhering to State and Federal regulations
• Formulate individualized nursing care plans using the nursing process
• Assess each resident daily, making adjustments to care plans as needed
