JOB DUTIES / KNOWLEDGE
- Following the plan of care or service plan as outline by the supervising nurse.
- Assist the patient with transfer, ambulating and prescribed exercise and instructed by the Home Care Nurse.
- Prepare modified diet as ordered by the Home Care.
- Observe and report specific vital signs and symptoms as instructed by Home Care.
- Remind the patient to take medications daily.
- Monitor skin changes daily, monitor changes and report to Agency RN.
- Perform incidental household functions necessary to maintain a safe E.g. comfort, cleanliness of the patient, and grocery shopping.
- Report changes in the patients' physical status to the Home Care Nurse or Home Care.
- Submit annual mandatory compliance documents such as annual TB questionnaire.
- Completes 12 hours of mandatory annual compliance training.
QUALIFICATIONS
- Must be a certified Home maker / Home Health Aide.
- Be a minimum of 18 years of age, able to read and write English.
- Bi-lingual is a plus.
- Have had a physical examination satisfying the New Jersey State Department of Consumers Affairs requirements for Home Health Care Workers.
