JOB DUTIES / KNOWLEDGE

• Personal care duties including bathing, grooming, feeding, turning and positioning.
• Assist the patient with transfer, ambulating, prescribed exercise, and instructed by the Home Care Nurse.
• When instructed by the Home Care Nurse, record weight, measure food and fluid intake and output.
• Prepare modified diet as ordered by the home care nurse.
• Observe and report specific vital signs and symptoms as instructed by Home Care
• Remind the patient to take medications specifically ordered by the physician, which is ordinarily self-
• Monitor skin changes daily, monitor changes and report to RN.
• Perform incidental household functions necessary to maintain a safe E.g. comfort, cleanliness of the patient, and grocery shopping.
• Report changes in the patients' physical status to the Home Care Nurse or Home Care Coordinator.
• Submit annual mandatory compliance documents such as annual physical, PPD, drug screen, a copy of x-ray report (if positive PPD) and annual TB questionnaire.
• Complete 12 hours of in-service every year.

QUALIFICATIONS

• Have completed a training program approved by the New York State Department of Health as a Home Health Aide or be enrolled as a trainee aide in New York State approved training program under the supervision of qualified RN.
• Be a minimum of 18 years of age, able to read and write English and to carry out
• Have had a physical examination satisfying the New York State Department of Health requirements for Home Health Care Workers.

Pay: Up to $21.00 per hour

License/Certification:

• PCA or HHA Certification (Preferred)
• We also provide Free PCA Training
