Home Health RN Case Manager

Job Summary
We are seeking a compassionate and dedicated RN Case Manager to join our team. In this role, you will provide high-quality nursing care to patients in their homes, ensuring they receive the support needed for recovery and well-being. You will be responsible for assessing patient needs, developing care plans, and collaborating with other healthcare professionals to deliver comprehensive patient care. Hours are 8a-5p M-F. All case managers also participate in an on-call rotation. Must have reliable transportation, valid driver's license, and vehicle insurance for light daily travel to patient's homes.

Duties

• Conduct thorough diagnostic evaluations of patients to determine their healthcare needs.
• Develop and implement individualized care plans based on patient assessments and discharge planning.
• Provide direct patient care, including administering medications, wound care, and monitoring vital signs.
• Educate patients and their families on health management, medication administration, and disease prevention.
• Ensure compliance with HIPAA regulations to protect patient privacy and confidentiality.
• Collaborate with interdisciplinary teams to coordinate managed care services and optimize patient outcomes.
• Utilize knowledge of physiology to assess patient conditions effectively and respond appropriately.
• Participate in utilization management activities to ensure efficient use of healthcare resources.
• Document all patient interactions accurately in accordance with organizational policies.

Qualifications

• Current Registered Nurse (RN) license in the state of Oklahoma.
• Experience in home health preferred, but not required.
• Strong understanding of discharge planning processes and managed care principles.
• Excellent communication skills and the ability to work collaboratively with patients, families, and healthcare teams.
• Knowledge of diagnostic evaluation techniques and patient care best practices.
• Ability to maintain professionalism while adhering to ethical standards in nursing practice.

Pay: From $30.00 per hour
