What are the responsibilities and job description for the Licensed Master Social Worker position at SUNY College of Optometry?
SUNY College for Optometry’s University Eye Center, an Article 28 diagnostic and treatment in midtown Manhattan, seeks to hire a Licensed Master Social Worker to manage the social service department. The University Eye Center has several specialized clinics and works with a wide range of patients including low vision, advanced care needs, traumatic brain injury, pediatric, special needs, older adults, diabetic and rehabilitation. The LMSW will be part of an interdisciplinary, patient-centered care team and will oversee coordination of behavioral health services. The successful candidate will have strong interpersonal, organizational, written and verbal communication skills.
The LMSW will be responsible for :
Clinical Services :
- Use an integrated behavioral health approach; provide bio- psychosocial- medical assessments, care coordination and referrals to social services for patients and their families.
- Conduct short-term and crisis interventions for individuals and families.
- Organize and facilitate groups for patients.
- Collaborate with optometrists to provide care to clinic patients.
- Develop care coordination plans for patients as needed.
- Supervise social work student interns.
- Provide tracking and follow-up with patients to ensure compliance and that any eligible benefits are successfully accessed.
- Educate medical staff and students on psychosocial issues related to patient care.

Compensation :

This is a full-time appointment. FLSA Exempt position, not eligible for the overtime provisions of the FLSA.
